Thank you for visiting the Mark IV™ Recall page. As we hope you can appreciate, we are trying to gather as much information as possible regarding the location and disposition of recalled firearms. Therefore, in the form below, we are asking that you share information regarding all Mark IV™ or 22/45™ pistols in your inventory. Additionally, if you can provide us with contact information for customers to whom you have sold Mark IV™ or 22/45™ pistols, there is a place for you to share that information below as well. This additional information will assist us in reaching out directly to the ultimate consumer of these pistols.

How to complete this form:

Please enter all Mark IV™ and 22/45™ pistol serial numbers that you have in inventory, as well as serial numbers of pistols that you have sold (to the extent you have that information available) and select the appropriate status for each serial number ("In Inventory" or "Dispositioned"). While serial number of dispositioned firearms are not required, if you provide us with that information, we will be able to reach out to end consumers more readily.

Our web portal will verify which "In Inventory" serial numbers are subject to the recall, and ask you to enter your contact information. We will send a shipping label when parts are available to complete the retrofit on these pistols.
After completing the sign up for all "In Inventory" serial numbers, if you provided any "Dispositioned" serial numbers that are subject to the recall, we will provide you a place to share customer information for the folks to whom you have sold these pistols. Again, while this information is not required, we would like to reach out to all Mark IV owners directly and sincerely hope that you will assist us in that endeavor.

When we are ready to retrofit your inventory of pistols, we will send you a prepaid shipping label and boxes (if needed).
When you receive your shipping label, simply follow the shipping instructions and return your inventory of pistols to us.
We will make every effort to return your pistols to you within one week of receiving them.

Please print and post the Product Safety Warning and Recall Notice at a prominent place in your store and, of course, help spread the word to your customers.
