gather as much information as possible regarding the location and disposition of recalled firearms. Therefore, in
the form below, we are asking that you share information regarding all Mark IV™ or 22/45™ pistols in your inventory.
Additionally, if you can provide us with contact information for customers to whom you have sold Mark IV™ or 22/45™
pistols, there is a place for you to share that information below as well. This additional information will assist
us in reaching out directly to the ultimate consumer of these pistols.

Serial Number Summary
Thank you for signing up for the Mark IV™ recall. You have been added to the list to have your
inventory of Mark IV™ pistols retrofitted. We will be retrofitting pistols on a first-come, first-served basis.
Here is what you can expect.
- When we are ready to retrofit your inventory of pistols, we will send you a prepaid shipping label and boxes
(if needed).
- When you receive your shipping label, simply follow the shipping instructions and return your inventory of
pistols to us.
- We will make every effort to return your pistols to you within one week of receiving them.

at a prominent place in your
store and, of course, help spread the word to your customers.

Thank you for signing your inventory of pistol up for the Mark IV™ recall. We have received your submission. Here is what you can expect:
- Due to the controls on the export and import of firearms and related parts and components, the retrofit of
your pistol will be handled by an authorized repair center in your country. There will be a delay before parts
are available.
- You may have already received communication from your local repair center.
- Once parts are available in your country, we will provide you with the repair center’s contact information.
You should reach out to the repair center and arrange to have your inventory of Mark IV pistols retrofitted
pursuant to the repair center’s schedule and requirements.
- The repair center will provide detailed instructions regarding how to prepare and ship your inventory of Mark IV pistols.
- The repair center will provide you with additional information as to expected turnaround time.
- In the meantime, we strongly recommend that you advise your customers not to use their Mark IV™ pistols
until they have been retrofitted.
